72 months of Home Health experience is required.

3 months of experience with Epic is required.

References

Candidates must provide 2 charge/lead references from within the past 24 months in order to be considered for this role

Coverage/Territory Info: "The travelers are assigned as our employees are assigned – to the entire territory every day. Once they are on board, our PT will help them make the best decisions on which patients to see to be the most efficient."

Territories Include: • North: Southern edge of Oshkosh, Eldorado, Rosendale, Van Dyne • South: Brownsville, Lomira, Mayville, Theresa, Horicon, Campbellsport, Bryon, Oakfield • East: St. Cloud, Mt. Calvary, Taycheedah, Calumet, Kiel, New Holstein • West: Ripon, Princeton, Markesan, Berlin • And everything in the middle

No Holidays Required

RTO Restrictions: Please request RTO up front if possible. Travelers may be required to participate in holiday and weekend rotation.

Guaranteed Hours: If confirmed to 3x12-hour shifts, SSM will guarantee a min. of 36 hours/ week. If confirmed to 4x12-hour shifts, SSM will guarantee a min. of 48 hours/ week. If confirmed to shift lengths less than 12-hours, SSM will guarantee a min. of 40 hours/week. If confirmed to shift lengths less than 12-hours, SSM will guarantee a min. of 40 hours/ week. Please note: SSM may cancel (call-off) the following per 4-week period: Up to 8 hours for Candidates confirmed to 8-hour shifts. Up to 10 hours for Candidates confirmed to 10-hour shifts. Up to 12 hours for Candidates confirmed to 12-hour shifts
