Requirements
Experience
24 months of role experience is required from any number of months.
24 months of Case Management experience is required from last 4 years.
Trauma Level 2 experience is required from any number of months
References
Candidates must provide 1 reference from within the past 3 years in order to be considered for this role
Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role
License
Candidates must have a California license

BLS (Required)

An interview may not be available prior to offer.

Candidates may not have been directly employed (full time, part time, or PRN) by any facility associated with CommonSpirit, including CHI and/or Dignity within the past year. Candidates who were employed PRN through outside firms will be considered.

Guaranteed Hours 40 hours per week.

EOW, First to Float.

BSN Required.

Must have prior Dignity experience or they will not consider*

patients seen/consults per day 15-20.

EMR: Interqual

Weekend Requirement every other depending on staffing. Would be at least one full weekend per month.

Holidays not required, but will work if they are willing.

Scrub Color/Uniform business causal attire.

Shifts depends on dept but hours are between 8-5pm.

Common duties/ Required skills or knowledge: Dealing with all payers, uninsured, homeless. Completing Initial assessments, IQ, DC planning, cerner.

Additional Notes Is this position UR or discharge planning or both? Both.

We will train candidates in IQ. Both DC Planning and UM are required.

Need 2 sup refs (charge accepted) at submittal; 1 w/in 12 mo. & 1 w/in 3 years worked.
