Experience

12 months of Home Health experience is required from last 3 years.

Experience with Oasis is required from any number of months.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

Candidates must provide 1 reference from within the past 3 years in order to be considered for this role

License

Candidates must have a Illinois license

BLS (Required)

Additional Details:

Scheduling: Expected to work every other weekend, every other holiday where applicable.

Block booking and Schedule patterns are based on department needs.

Guaranteed Hours: 40 hours per week

Home Health experience including OASIS documentation, EMR proficiency, IV and Wound care, Phlebotomy, Catheter care

Active car insurance and dependable vehicle.

Types of procedures required.
RN Direct patient care provided to patients in their home- Vital Signs, Physical Assessments including OASIS, IV, Wound, Lab draws, Disease Management education, Case Management

Home Health experience including OASIS documentation, EMR proficiency, IV and Wound care, Phlebotomy, Catheter care.

We have a census of 550-600 patients, RN would be responsible for completing 5-6 visits daily, or 25 per week.

Nurse to patient ratio of this unit. 20 or 25-1

Dress Code: Scrubs, Professional appearance, tattoos must be covered, no facial piercings, conservative dress where no midriff is exposed, undergarments are not visible beneath shirts.
