Requirements
Experience
12 months of role experience is required from last 3 years.
12 months of Hospice experience is required from last 3 years.
Experience with Homecare Homebase (HCHB) is required from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role
License
Candidates must have a Wisconsin license or compact license
Certifications
BLS (Required)
Proof of Identification
Driver's license required

Additional Details:
Schedule: every weekend on Friday at 5pm and end on Monday at 8am.
Territory: Racine county, Kenosha county, South Milwaukee, Burlington, Waterford and surrounding areas. MKE team takes over at Rawson and the Racine team goes up to the Illinois state line
Length of Contract: 12-16 weeks
Any call?: The position is strictly an on-call role, scheduled visits, admissions, deaths, etc.
