Additional Details:
A minimum of 1 year acute care experience is required to work in acute care facilities. The following are considered acute care facilities: Hospital (General Acute Care as well as Psychiatric, Specialized and Rehabilitation Hospitals, and Long Term Acute Care or LTAC), and Ambulatory Care Facilities.
Orientation is weekly, on Mondays.

Weekly Guaranteed Hours: 40
Weekend Hours: Every Weekend.
CCM/ACM preferred.
Additional Notes: InterQual, discharge planning, Conditions of Participation, CMS guidelines, required. MorCare preferred.
Patient Population: Geriatric, adult, and adolescent, women, and infant
Must Have Skills/Experience: Utilization Management, Utilization Review (InterQual), Discharge Planning, insurance, acute hospital case management experience, recent traveler dyad case management experience
Preferred Skillset/Experience:
Common Diagnosis/Treatment: CHF, Sepsis, COPD, Pneumonia, UTI, CVA, Acute Renal Failure
Required Education: Associate’s degree, Bachelor of Nursing preferred.
