12 months of Physical Therapy experience is required from last 3 years.

Candidates must provide 2 manager/supervisor references from within the past 2 years in order to be considered for this role

Is Home Health involved? Occasionally- up to 5/day; Radius of coverage- 40 miles
Appropriate education level required in accordance with state licensure.
