Additional Details:
A minimum of 1-year acute care experience is required to work in acute care facilities. The following are considered acute care facilities: Hospital (General Acute Care as well as Psychiatric, Specialized and Rehabilitation Hospitals, and Long-Term Acute Care or LTAC), and Ambulatory Care Facilities.

Equipment: Laptop, two monitors, desk and phone provided by facility
Must Have Skills/Experience: Discharge planning in the acute hospital setting.
ACM or CCM preferred. Creating discharge plans, based on insurance benefit, knowledgeable of utilization reviews, coordinating services to prevent readmission. Knowledgeable of CMS regulation and Virginia Medicaid
Patient Population- geriatric, adult, peds, newborn.
Common Diagnosis/Treatment: Cerebral Vascular Accident, Diabetes, Heart and Vascular, Sepsis
Dress Code: Business Causal attire
Orientation is bi-weekly on Tuesdays
Danville Facilities require consent for to random drug screening and must complete a Criminal Questionnaire/Consent Form. Random drug screens are completed on site.
