Requirements

Experience

24 months of role experience is required.

3 months of travel experience is preferred.

24 months of Case Management experience is required.

3 months of experience with Epic is required.

References

Candidates must provide 2 references in order to be considered for this role

License

Candidates must have a New York license

DCP Experience in an Acute hospital facility

Must be able to take directions from a highly experienced administrator social worker

Interqual

Will work with medical patients of all ages except Pediatrics doing discharge planning only. Inpatient Case Management with Complex patients

Preferred Experience: PRI (Patient Review Instrument), Teaching facility experience
